Your life, your services survey 2019

Your answers will help improve care and quality of life for everyone affected by Parkinson's.

We’re once again asking everyone affected by Parkinson’s to complete the Your life, your services survey. The annual survey helps us build a picture of the state of Parkinson’s health and social care services and to better understand your individual needs.
We’d really appreciate it if you could take the time to complete the 2019 survey. This will mean we can compare results with previous years and keep focusing on the areas that need improvement. We want to know if people affected by Parkinson’s are getting the right health and support services. If we know where improvements are needed we can target these areas.
We also have some questions about how you feel in terms of how you manage living with Parkinson’s day to day.
We need as many people as possible to fill in the survey. Completing all its parts can help make sure everyone gets the right services and support.
All the questions are directed at people who have a diagnosis of Parkinson’s themselves. But feel free to ask someone to help you complete the survey if you prefer.

Your responses are confidential and will only be used anonymously by Parkinson’s UK to present an overall picture of service satisfaction. We ask for your postcode within this survey to accurately capture information for specific areas but all responses remain anonymous, with data only available to the team working on this particular project. All surveys responses are aggregated to further protect anonymity.
